SMBA Volunteer Disclosure Form
This form is required to be completed by all coaches, managers, treasurers, trainers, and SMBA Board Members as a declaration of completion of volunteer requirements and OBA requirements, if applicable.

Please use this section to identify the positions you are interested in volunteering for. Note: If there is a child (yours, or someone else's) that you would like to be associated with in terms of your volunteer position, please provide their first and last name below.

VOLUNTEER REQUIREMENTS

All volunteers with SMBA (coaches, managers, trainers, convenors, etc.) are required to complete the following training. *Note: any volunteer that is not working directly with children/youth and/or financial records are not required to complete the below requirements (e.g., scheduling volunteers, webmaster). 

To complete the below requirements, volunteers must download the OBA OnDeck app (and create an account) and have a NCCP# from The Locker (coach.ca). 

1) Volunteer Screening Level 1 (includes Volunteer Screening Disclosure Form, Abuse Awareness for Adults, Safe Sport Module through CAC). Note: The Safe Sport Module requires you to have an NCCP# (hockey coaches will already have an NCCP #, all new registrants will need to register for an NCCP # before proceeding with the Safe Sport training). 
2) Volunteer Screening Level 2
3) Vulnerable Sector Check/ E-Pic
4) For Shamrocks coaches ONLY - Coaches and managers in the rep Shamrocks program are required to complete additional training which is outlined on Ontario Baseball Association's OnDeck app and at Baseball Canada.
5) Read and agree to follow Baseball Ontario's Safe Supervision Policy Rule Two (Supervision_Policy_06-23-2019).

*Requirements must be completed on Baseball Ontario's OnDeck App and proof uploaded below. See below for screenshots of information to be uploaded from the OnDeck App after completion.

Note: SMBA will only reimburse volunteers for the required training for their respective position (please submit using the expense form on our website). If a volunteer choses to complete additional training, they will be solely responsible for the cost (e.g., if a 9U rep coach takes an infielding clinic they will be personally responsible for the cost as it is not a requirement of a 9U rep coach).

Once E-PIC has been completed and reviewed, a certificate will be available for download - provide downloaded screening.

Volunteer Requirements

Requirement #5: Volunteers shall also review and agree to follow Baseball Ontario's Safe Supervision Policy Rule Two (Supervision_Policy_06-23-2019).
