PERMISSION AND RELEASE OF LIABILITY AND PARENTS/GUARDIANS INDEMNIFYING AGREEMENT

I hereby grant permission for my child to participate in the activities of the Sudbury Minor Baseball Association (SMBA). I am aware of the risks involved in playing baseball. Therefore, I will not hold SMBA, its organizers, officials, members, directors, or any other persons connected with SMBA, liable for any injuries occasioned to my child during, or as a result of their participation in, the activities of SMBA, or while traveling to or from such activities. I hereby waive any rights against the SMBA, its organizers, officials, members, directors, or any other person or persons connected with it, and release them from any costs, claims, and/or liabilities.

As the parent/guardian of the child registered above, a child under the age of eighteen (18) years, agree to not make any claim or take any proceedings against any other person or corporation who might claim contribution or indemnity under the provisions of any statute or otherwise against the SMBA, its organizers, officials, members, directors, or any other persons connected with SMBA. I agree at all times to indemnify and save harmless the SMBA, its organizers, officials, members, directors, or any other persons connected with SMBA, against, and from, any and all claims, demands, actions, and claims for contribution or indemnity whether under and statute or otherwise which may be made or brought by me or other persons on behalf of the said child or in any way arising out of injuries suffered by said child as a result of, or arising out of, engagement in the activities of SMBA directly or indirectly, including the costs of defending the said action or claim.
